COVID-19 MANAGEMENT PROTOCOL

Prophylaxis
While there is very limited data (and none specific for COVID-19), the following “cocktail” may have a
role in the prevention/mitigation of COVID-19 disease. While there is no high-level evidence that this
cocktail is effective; it is cheap, safe and widely available.
Vitamin C 500 mg BID and Quercetin 250-500 mg BID
Zinc 75-100 mg/day (acetate, gluconate or picolinate). Zinc lozenges are preferred. After 1
month, reduce the dose to 30-50 mg/day.
Melatonin (slow release): Begin with 0.3mg and increase as tolerated to 2 mg at night
Vitamin D3 1000-4000 u/day
 
Symptomatic patients (at home):
Vitamin C 500 mg BID and Quercetin 250-500 mg BID
Zinc 75-100 mg/day
Melatonin 6-12 mg at night (the optimal dose is unknown)
Vitamin D3 1000-4000 u/day
Optional: ASA 81 -325 mg/day
Optional: Hydroxychloroquine 400mg BID day 1 followed by 200mg BID for 4 days
Optional: In highly symptomatic patients, monitoring with home pulse oximetry is recommended
Mildly Symptomatic patients (on floor):
Vitamin C 500mg q 6 hourly and Quercetin 250-500 mg BID (if available)
Zinc 75-100 mg/day
Melatonin 6-12 mg at night (the optimal dose is unknown)
Vitamin D3 1000-4000 u/day
Enoxaparin 60 mg daily
Methylprednisolone 40 mg daily; increase to 40mg q 12 hourly in patients with progressive
symptoms and increasing CRP
Famotidine 40mg daily (20mg in renal impairment)
Optional: Hydroxychloroquine 400mg BID day 1 followed by 200mg BID for 4 days
Optional: Remdesivir, if available
N/C 2L /min if required (max 4 L/min; consider early t/f to ICU for escalation of care).
Avoid Nebulization and Respiratory treatments. Use “Spinhaler” or MDI and spacer if required.
Avoid non-invasive ventilation
T/f EARLY to the ICU for increasing respiratory signs/symptoms and arterial desaturation.
 
 
q12H means every 12 hours.
PO” means the medication is taken by mouth “bid” or twice a day.

Some have even banned reusable bags. Why? Because, as we knew even before they banned them, single-use plastic bags are more hygienic. 97% of people never wash their reusable bags. But, environmentalist groups call virus fears from reusable bags “misinformation". Alex Truelove of the Public Interest Research Group says, “there are no studies … that reusable bags are transmitting viruses.” He’s right about the lack of human COVID-19 studies. But, in 2013, when millions of piglets died of a novel swine coronavirus, the Agriculture Department concluded that reusable feed bags were the likely cause. After the virus fears diminish, politicians will put their plastic bag bans back in place. Why? “8 million metric tons of plastic enters the ocean every year,” explains Truelove. Again, that’s true, and plastic pollution is a serious problem. But economics professor Ross McKitrck of the University of Guelph pushes back, saying, “Get some facts on the table about where that plastic comes from.” Almost all of it comes from Asia and Africa. The North American fraction is less than 1%. Banning cheap and convenient grocery bags may make politicians feel good, but when it comes to pollution, it won’t make a difference.

Tara Reade has accused current presidential candidate (and former Vice President) Joe Biden of sexually assaulting her when she worked in his Senate office in 1993. If true, it was unambiguous assault by Biden. According to Reade, he began orally kissing her without her permission, pushed her up against a wall, reached up under her skirt, and roughly penetrated her vagina with his fingers. "He said 'come on man, I heard you liked me,'" Reade recalled in an interview. "For me, it was like, everything shattered. I looked up to him, he was like my father's age, he was this champion of women's rights, in my eyes. I couldn't believe it was happening. It was surreal." Reade had previously complained to her bosses about sexual harassment in Biden's office - a supervisor had asked her to serve drinks at an event because she had attractive legs. In 2019, Reade said that there was rampant sexism among Biden staffers but that the problem was not Biden himself. The recent precedent set by the mainstream media and leftists during the Kavanaugh horror is this: All accusations should be reinvestigated, no matter how old. Blasey Ford’s accusations against Kavanaugh were far less credible – she accused Kavanaugh of actions from his teenage years in the early 1980s. Reade’s accusations concern an explicit sexual assault that Joe Biden committed while he was a sitting U.S. senator. Joe Biden himself has publicly taken the position that all women should be believed, no matter how much time it takes for them to come forward. The whole #metoo movement is being revealed as a leftist “dirty tricks” campaign. Actress – and Biden supporter – Alyssa Milano says she never thought #MeToo would ‘destroy innocent men.’” "I believe that even though we should believe women, and that is an important thing, what that statement really means is that for so long, the go-to has been not to believe them," said Milano. "We have to societally change that mindset to believing women, but that does not mean at the expense of not giving men their due process and investigating situations.
